PARENT OR GUARDIAN CONSENT (version 2026-10-19)
Event: Mock event, Tuesday, October 20, 2026, 10:20 AM – 11:30 AM, My house. Organized by Alas, Formación & Apoyo, with Diego Carlson.
Minors: (the minors named above).
1. I am the parent or legal guardian of the minors named here and have the authority to sign for them.
2. I consent to their taking part in this event.
3. I understand this is an educational event. It is not medical care, diagnosis or treatment.
4. The information in this reservation (ages, allergies or needs, and the emergency contact) is accurate. I will tell the organizers about anything important.
5. The minors will follow the organizers’ instructions. The organizers may ask a participant to leave if their behavior puts anyone’s safety at risk. I am responsible for the minors getting to the event and getting home when it ends.
6. In an emergency, I authorize the organizers to call the emergency contact and, if they can’t reach me, to seek emergency medical care for the minor. The cost of that care is my responsibility.
7. Photos are handled according to the choice made in this reservation.
8. To the extent the law allows, I release Alas, Formación & Apoyo, Diego Carlson and the event volunteers from claims for ordinary injuries arising from taking part, except those caused by gross negligence or intentional misconduct.
9. Typing my full name and checking the box to agree is my electronic signature and has the same effect as a handwritten signature.
